The Portage County Court of Common Pleas authorized this Notice. You are not being sued.

This is not a solicitation from a lawyer.

  • A Settlement has been reached in a class action lawsuit concerning Coleman Professional Services, Inc. (“Defendant” or “Coleman”) and a data incident (the “Data Incident”) that occurred between September 18 and October 31, 2023, when one or more unauthorized individuals allegedly accessed information on Coleman’s email accounts, including names, dates of birth, Social Security numbers, driver’s license or state issued ID numbers, financial account information, treatment/diagnosis information, patient identification numbers, and health insurance/payment information (collectively, “Private Information”).
  • The lawsuit is called Armstrong, et al., v. Coleman Professional Services, Inc., No. 2024CV00139 (Portage County Common Pleas). The lawsuit asserts claims related to the Data Incident (the “Litigation”). The Defendant in the Litigation is Coleman. Coleman denies any wrongdoing or that it is or can be held liable for the claims made in the Litigation. The Settlement does not establish who is correct but rather is a compromise to end the Litigation.
  • Members of the Class are all United States residents whose Private Information was implicated in the Data Incident, including all United States residents to whom Coleman Professional Services, Inc. mailed direct notice of the Data Incident.
  • All Class Members are eligible to submit claims for identity protection services and cash payments of roughly $70 (assuming a claim rate of 10% of the roughly 51,889-person Settlement Class), which will be pro rata increased or decreased based on the money still remaining in the Settlement Fund at the time of payment. In addition to these benefits, you are eligible to be reimbursed for any documented losses or expenses (up to $5,000 total) that you can demonstrate are traceable to the Data Incident.
  • If you are a Class Member, your legal rights are affected regardless of whether you act or do not act. Please review this website carefully and continue to monitor it for updated information.

YOUR LEGAL RIGHTS AND OPTIONS IN THIS SETTLEMENT

Submit A Claim Form

This is the only way you may receive benefits from this Settlement. The deadline to submit a Claim Form is October 5, 2026. 


Exclude Yourself from the Settlement

You will receive no payment, but you will retain any rights you currently have with respect to Defendant and the issues in this case. The deadline to exclude from the Settlement is September 4, 2026. 


Object to the Settlement

Write to the Court explaining why you do not agree with the Settlement. The deadline to object is September 4, 2026.


Attend the Final Approval Hearing

You may ask the Court for permission for you or your attorney to speak about your objection at the Final Approval Hearing. The Final Approval Hearing will be held on October 13, 2026 at 9:45am ET.


Do Nothing

You get no payment and you give up rights.

  • The Court in charge of this case still has to decide whether to approve the Settlement. No Settlement benefits or payments will be provided unless the Court approves the Settlement and it becomes Final.
